FAQ
Where can I find all current North Carolina Secretary of State business forms?
Current business-entity forms are listed on the Business Registration forms page. The page is organized by filing family and shows the form number, title, fee, and the available online filing path for each item.
North Carolina’s current business filing fees are listed on the Business Registration fee page. Online filings route through Business Creation, the Business Registration filing search, and Reinstatements and Requalification, depending on the filing type.
Is the registered agent consent form required to be filed with the North Carolina Secretary of State?
North Carolina does not publish one separate statewide registered-agent consent form for ordinary business-entity filings.
Instead, consent is embedded in the main filing or supplied as a separate written consent attached to that filing. For example, Designation of Registered Office Address and/or Registered Agent (Form BE-05) states that the designated registered agent’s written consent appears on the form, and it also allows separate written consent to be attached.
The Registered Office and Registered Agent Information page states that a registered agent must agree to act as registered agent.

Most entities use Statement of Change of Registered Office and/or Registered Agent (Form BE-06), which carries a $5.00 filing fee. A filing that also changes principal-office information may use Designation Statement of Change of Principal Office and Registered Agent Name/Office (Form BE-17), which carries a $10.00 filing fee.
An unincorporated nonprofit association uses Amendment of Statement of Appointment of Agent (Form NA-02), which carries a $5.00 filing fee.

What is the difference between the entity-filed change form and the agent-filed change form in North Carolina?

The agent-filed form is Form BE-12. It is filed by the continuing registered agent when the agent changes its own office address for represented entities and does not substitute a new agent.
The agent-filed form is the multi-entity filing method for one agent address change. The form requires written notice to each represented entity before filing.
